Roster Entry Form
A Roster Entry Form must be completed and submitted online to begin the assessment process. A separate form must be completed for each class.
The Roster Entry Form provides the Blanchard Learning Center with the information they will need to distribute the assessment. Classes for both online and pen/paper assessments must use the online Roster Entry Form.
Step 1
Fill in each section of the Roster Entry Form.
Step 2
Print and keep a copy of the Roster Entry Form and any special instructions for yourself. Send a copy of the Roster Entry Form to the class facilitator.
Step 3
Submit the Roster Entry Form before distributing assessment packages.
If the Roster Entry Form submitted is incomplete or incorrect, your Learning Services Coordinator may need to contact you and your order my be delayed until issues are resolved.
